Overview
PRAPARE is not a software tool itself, but a standardized screening tool and protocol developed by the National Association of Community Health Centers (NACHC). It consists of a set of national core measures as well as optional measures that can be used to screen patients for a wide range of social needs. Many EHR vendors and SDOH software platforms have integrated the PRAPARE questionnaire into their systems.
